About The Position

Community Health Workers (CHW) serve as trusted frontline public health workers who deeply understand the communities they support. Acting as a vital link between individual, healthcare services, and social support systems, CHWs play a key role in: Facilitating access to care, improving delivery service, and enhancing health outcomes. Requirements

• Minimum of High School Diploma.
• Minimum of 1 year in the maternal child health field.
• Must be able to speak and write fluently in Spanish and English.
• Computer proficiency required with Microsoft programs.
• Strong interpersonal & oral/written communication skills required.
• A valid Driver’s license with the ability to provide proof of ongoing automobile insurance coverage is required.
• This position will entail reimbursable local and statewide travel utilizing personal vehicles.

Nice To Haves

• Bachelor's degree (BA/BS) in Public Health or related field Preferred.
• A strong knowledge of their respective counties and the New Jersey social service system and experience in the field of public health, specifically maternal child health, is strongly preferred.
• Knowledge and experience relating to the field of non-profit.

Responsibilities

• Conducts community outreach and assists women and their families in accessing healthcare and social services by offering resources and referrals.
• Provides case management and assists with care coordination. CHWs provide virtual and/or home visits to support maternal and child health clients.
• Follows up and assess clients' needs and provides accurate documentation of information in the Unite Us system.
• Conducts community education sessions and workshops and will maintain a regular presence in the community by participating in health fairs and events.
• Collaborate with community-based organizations to ensure a comprehensive system of care.
• Participates and assists with the implementation of quarterly Community Advisory Board meetings.
• Works with other community-based supportive services/agencies throughout the county to provide a comprehensive system of care to the families served
• Other duties may be assigned as needed.
