About The Position

The Community Health Worker (CHW) – Alameda County serves a dual role as both a direct service provider to community members and the primary on‑the‑ground anchor for Homebridge’s Community Health Worker services in Alameda County under the Alameda Alliance for Health (AAH) contract. In addition to providing high‑quality, person‑centered CHW services to clients, this role plays a critical role in launching, operationalizing, and scaling Homebridge’s CHW footprint in Alameda County. The CHW – Alameda Anchor will support “building the business” for Homebridge by developing a deep understanding of the AAH contract, CHW program requirements, documentation and reporting standards, and county‑specific systems. This role will help translate contract requirements into day‑to‑day workflows, support early implementation, and serve as a key liaison between field operations and central leadership.

Requirements

  • High school diploma or equivalent required
  • State-recognized CHW/P/R Certificate or at least 2000 hours/1 year of full-time work as a CHW/P/R required

Responsibilities

  • Provide direct CHW services to clients, including outreach, engagement, health education, care navigation, and care coordination.
  • Support clients in accessing and navigating medical, behavioral health, and social service systems.
  • Conduct assessments, screenings, and follow‑ups aligned with CHW and Medi‑Cal/CalAIM program guidelines.
  • Assist clients with appointments, referrals, documentation, and follow‑through on care plans.
  • Build trusting relationships with clients using trauma‑informed, culturally responsive, and strengths‑based approaches.
  • Advocate on behalf of clients to address barriers to care, housing stability, and overall well‑being.
  • Document all services accurately and timely in required systems to meet contract and compliance standards.
  • Serve as Homebridge’s primary CHW point‑of‑contact for Alameda County service delivery during the initial contract and program launch phase.
  • Develop deep working knowledge of Alameda Alliance for Health (AAH) contract requirements, CHW guidelines, service definitions, and reporting expectations.
  • Support translation of contract and MCP requirements into day‑to‑day CHW workflows, tools, and practices.
  • Identify early implementation gaps, risks, or operational challenges and elevate them proactively to leadership.
  • Support continuous refinement of intake, care pathways, documentation, and service delivery processes for Alameda CHW services.
  • Contribute to development of SOPs, job aids, workflows, and training resources related to Alameda CHW operations.
  • Support accurate and compliant documentation to meet AAH, Medi‑Cal, and CHW program requirements.
  • Assist with tracking service activities, outputs, and required data elements for internal monitoring and external reporting.
  • Participate in quality improvement efforts by identifying trends, challenges, and opportunities in service delivery.
  • Support preparation for audits, reviews, or MCP check‑ins as requested by leadership.
  • Ensure services are delivered in alignment with Homebridge policies and applicable county, state, and MCP requirements.
  • Build and maintain strong working relationships with Alameda County providers, community‑based organizations, and referral partners.
  • Support coordination with AAH, clinical partners, and care teams to ensure integrated, high‑quality client care.
  • Represent Homebridge professionally in meetings, case conferences, and collaborative spaces as appropriate.
  • Support outreach and relationship‑building efforts to grow referral pathways and program visibility in Alameda County.
  • Provide field‑based insights to leadership to inform program design, staffing models, and future scaling of CHW services.
  • Support onboarding and peer guidance for future Alameda‑based CHWs as the program grows.
  • Contribute to early “build and stabilize” efforts that position Alameda CHW services for long‑term sustainability.
  • Perform other related duties that support program launch, stabilization, or expansion as assigned.
