Aspire Indiana Health•Carmel, IN

About The Position

Aspire Indiana Health is a nonprofit provider of comprehensive whole-health services, including primary medical care, behavioral health, recovery services, and programs addressing social drivers of health such as housing and employment. It operates health centers in four Central Indiana counties and serves people of all ages and walks of life. Community Health Workers (CHWs) are flexible members of a health care team who work with vulnerable populations to improve health outcomes. The role aims to improve community health by increasing health knowledge and self-sufficiency through outreach, community education, informal counseling, social support, and advocacy. The Community Health Worker - Transportation provides and/or coordinates transportation services for clients and employees and coordinates maintenance of Aspire’s fleet. Employees must complete a drug screen, TB test, and extensive background checks. Aspire requires employees to be fully vaccinated against COVID-19 before joining and strongly encourages a flu shot.
