About The Position

A Community Health Worker at Oak Street Health serves as a bridge between patients, the community, and medical systems to remove barriers and improve wellness. The CHW advocates for and liaises with patients through proactive in-person and phone outreach, promoting health literacy and connecting patients with resources that support healthier lives. The role requires flexibility, problem-solving, strong communication, and knowledge of the community served. CHWs work closely with Medical Social Workers and the wider care team to manage patient care plans, support care decisions, and coordinate clinical and complementary services. The position works within the CHW’s scope of practice and does not require a clinical license. CHWs meet patients in locations suited to their needs and preferences, including health centers, homes, medical facilities, and community settings. About half of the work is expected to take place outside the center, so reliable transportation is required. The role also involves timely, confidential documentation in the electronic medical record and participation in interdisciplinary team meetings.
