About The Position

Community Health Worker (School-Based Health Center) Make a Difference in the Lives of Students and Families at TCAPS. We are seeking a compassionate, community-focused Community Health Worker (CHW) to join our School-Based Health Center team at TCAPS. In this role, you will serve as a trusted connection between students, families, schools, healthcare providers, and community resources to improve access to care and support positive health outcomes. The ideal candidate is passionate about working with children, adolescents, and families, enjoys building relationships within the community, and is committed to reducing barriers to health and wellness. The Community Health Worker supports students and families by providing outreach, education, care coordination, resource navigation, and advocacy services. Working closely with school personnel, healthcare providers, and community partners, the CHW helps connect individuals to healthcare, behavioral health, insurance coverage, and community resources while promoting wellness and addressing social determinants of health. This position also supports program quality improvement initiatives, community engagement activities, grant requirements, and compliance with federal, state, and organizational regulations.

Requirements

  • High school diploma or equivalent.
  • Experience working with children, adolescents, families, or community-based populations.
  • Strong interpersonal, communication, and relationship-building skills.
  • Ability to work collaboratively with diverse populations and community partners.
  • Proficiency with computers and electronic documentation systems.
  • Valid driver's license and reliable transportation, if required.

Nice To Haves

  • Community Health Worker certification or willingness to obtain certification.
  • Associate's or bachelor's degree in public health, social work, human services, healthcare, education, or a related field.
  • Experience in school-based health, community health, care coordination, behavioral health, or social services.

Responsibilities

  • Build trusting relationships with students, families, and community members.
  • Serve as a liaison between students, families, healthcare providers, and school personnel while maintaining confidentiality.
  • Assist students and families in accessing healthcare, behavioral health, and community support services.
  • Coordinate referrals and follow-up activities to ensure continuity of care.
  • Help individuals enroll in health insurance programs and access available benefits and resources.
  • Address barriers related to transportation, housing, food security, healthcare access, and other social determinants of health.
  • Conduct outreach to students and families through phone, electronic, school-based, and community-based engagement activities.
  • Promote awareness of School-Based Health Center services and available community resources.
  • Participate in school and community events that support health education and wellness initiatives.
  • Assist with re-engaging students and families who have missed appointments or become disconnected from care.
  • Provide health education, supportive guidance, and advocacy to students and families.
  • Distribute approved educational materials and promote healthy behaviors.
  • Support training and education efforts related to confidentiality, youth healthcare access, and school-based health services.
  • Participate in interdisciplinary team meetings, care planning discussions, and quality improvement activities.
  • Coordinate patient satisfaction surveys and support continuous quality improvement initiatives.
  • Facilitate Community Advisory Council activities, including meeting planning, communication, and documentation.
  • Assist with grant reporting, program evaluation, and compliance activities.
  • Maintain accurate and timely documentation in accordance with organizational policies.
  • Protect student and patient confidentiality in compliance with HIPAA, FERPA, State of Michigan laws, and organizational requirements.
  • Support reporting requirements, state reviews, and site visit activities.
