About The Position

The School-Based Community Health Liaison functions as the primary connection between the partnering school district(s) and Four Rivers. This role is responsible for helping students and families of the partnered school district(s) navigate and access community services and adopt healthy behaviors. The liaison will act as a bridge between the school district and the clinic, coordinating school-based medical, dental, and community resource clinics and events. They will also assist eligible clients in applying for publicly sponsored healthcare insurance plans, conduct community outreach to promote health, provide information on available resources, offer social support and informal counseling, and advocate for individual and community health needs. Data collection for identifying community health needs and continuous process improvement are also key aspects of this role, ensuring professional duties align with the mission, vision, and values of both Four Rivers Community Health Center and Rolla Public Schools.

Responsibilities

  • Functions as the primary connection between the partnering school district(s) and Four Rivers.
  • Helps students and families of the partnered school district(s) navigate and access community services and adopt healthy behaviors.
  • Acts as a liaison between the school district and the clinic to schedule and coordinate all school-based medical, dental, and community resource clinics and events.
  • Assists eligible clients to apply for publicly sponsored health care insurance plans providing close follow up throughout the approval process.
  • Conducts outreach in the community that promotes, maintains, and improves individual and community health.
  • Provides information on available resources, provides social support and informal counseling, and advocates for individuals and community health needs.
  • Collects data to help identify community health needs and conducts continuous process improvement as part of the quality assurance program.
  • Establishes trusting relationships and interacts professionally with partnering school district staff, community partners, healthcare providers, social service providers, students/patients and their families.
  • Contacts prospective program participants by mail, telephone or in person to inform them of available services.
  • Explains the impact the program would have on the participant and encourages participation.
  • Coordinates the obtainment of pre-working school-based consent and registration forms and reliably enters information into the patient's charts.
  • Schedules, coordinates, and attends school-based events and child centered clinic events planned by Four Rivers and/or the partnering school district, as well as any applicable community meetings/events.
  • Schedules and executes school-based appointments such as dental exams, well-child checks, sports physicals, immunizations, etc.
  • Provides appropriate follow-up and referrals following a school-based medical, dental or community resource event, working directly with a patient's treatment team for care coordination and/or case management.
  • Assists families, groups and communities in accessing health related services.
  • Assesses individuals' eligibility for and assists in applying to services such as financial assistance, insurance and other programs, providing necessary follow-up, tracking and data collection to assure continuous enrollment.
  • Utilizes the electronic healthcare record, excel spreadsheets, word processing and other software programs to format documents, spreadsheets, brochures, flyers, newsletters, training materials and other documents.
  • Produces technical graphics, charts and tables, selecting the format that will best display the material.
