About The Position

The Community Health Worker (CHW) works directly with patients, families, community partners, and interdisciplinary clinical teams to improve health outcomes, reduce barriers to care, and support equitable access to healthcare and social services. They serve as a trusted liaison between patients, healthcare providers, social service agencies, and community organizations, delivering culturally responsive health advocacy, health education, and health navigation services to Medicaid members with chronic conditions, unmet health-related social needs, or other qualifying needs. The CHW helps patients build self-management skills, navigate healthcare systems, maintain insurance coverage, address social determinants of health, and successfully connect to medical, behavioral, and community-based resources.

Requirements

Associate's degree required
Completion of Community Health Worker training or qualifying work experience.
Strong interpersonal, communication, and organizational skills.

Nice To Haves

Bachelor's degree preferred.
Experience working with underserved populations preferred.
Bilingual English/Spanish preferred.

Responsibilities

Promote a safe, cooperative, and professional healthcare environment to ensure optimum patient care.
Adheres to all hospital policies, procedures, rules, and regulations.
Makes Customer Service a priority, treating patients, visitors, community partners, and co-workers in a professional manner exercising courtesy, respect, and cultural humility.
Demonstrates standards of performance and behaviors consistent with the DRIVE to Patient-Centered Excellence.
Deliver Community Health Worker services as assigned, including but not limited to: Health Advocacy, Health Education, Health Navigation, Social Needs Support, Population Health Program Support.
Collaborate with physicians, nurses, social workers, care managers, financial counselors, and community-based organizations to coordinate services and improve continuity of care.
Document all CHW encounters accurately in the electronic medical record in accordance with Medicaid documentation requirements.
Maintain patient tracking systems, referral logs, outreach records, and quality reporting documentation.
Participate in quality improvement activities, workflow development, and population health performance initiatives.
Contribute to educational materials, workflows, patient communication campaigns, and program reporting.
Attend scheduled department, division, supervision, and training meetings and participate as appropriate.
Perform other duties at the discretion of leadership.
