Requirements
- Associate’s degree. Experience in a health care setting may be substituted year for year for the education requirement.
- One (1) year experience in a health care setting, community health education and/or outreach position.
- Fluency in English is required.
- Proficient computer skills.
Nice To Haves
- Additional fluency in Spanish is preferred.
Responsibilities
- Facilitates access to ICHS school-based health centers and primary care clinics to connect youth and their families to quality affordable care.
- Supports referrals and linkages to external and internal services for school-based patients.
- Provides operational support for the school-based health center.
- Identifies and maintains relationships with school partners who serve populations with identified health disparities.
- Promotes access to preventive care through independent outreach and education with individuals or groups in the community.
- Provides support with insurance enrollment.
