About The Position

This position has multiple responsibilities to support the care coordination of people experiencing homelessness primarily through in-field (1) medical care coordination, (2) dental care coordination, (3) behavioral health care coordination, and (4) eligibility support. In addition, this position will include other administrative and coverage tasks for Saban Community Clinic’s Homeless Services Program as needed.

Requirements

  • High School Diploma required
  • Minimum 1-3 years of program administration, Community Health Worker, or Care Coordination experience
  • Excellent customer service skills
  • Committed to working with the homeless and disconnected population
  • Must be flexible and able to pivot / multi-task, especially when in the field.
  • Ability to maintain confidentiality in all aspects of the work environment.
  • Basic computer knowledge; ability to use computers, tablets, cellphone for record keeping
  • Attention to detail and accuracy in medication delivery
  • Current California driver’s license and state-required auto insurance when using personal vehicle on Clinic business
  • Strong time management and organizational skills
  • Adaptability, effective communication, and the ability to de-escalate and resolve conflict

Nice To Haves

  • Bilingual in Spanish a plus
  • case management or outreach experience a plus

Responsibilities

  • Supports patients with insurance eligibility and enrollment questions including primary care switches, insurance applications, and referrals.
  • Links Homeless Program patients to a medical & dental home as well as community resources and services to support their overall wellness and recovery.
  • Follows Clinic safety and de-escalation protocols in the field.
  • Corresponds directly with internal and external care teams to develop care plans for patients
  • Assists with specialty medical, behavioral health and dental care referrals (including follow-up and transportation support).
  • Provides in-field support including visits to partner sites to support patients with registration forms, care questions, and insurance questions.
  • Supports patient access to medication, pharmaceutical supplies and related referrals in coordination with the care team.
  • Provides coverage support when necessary for the shower program and the mobile unit.
  • Completes incident reports in a timely manner.
  • Maintains professionalism, confidentiality, and trauma-informed engagement when interacting with patients.
  • Supports the administrative and program coordination needs of the Homeless Services programs.
  • Performs other duties as assigned by supervisor/manager.
