The Community Health Worker (CHW) Program Manager is responsible for supporting community health activities by overseeing vendor/partner organization, coordination of patient support with providers and care teams, coordinating activities related to logistics, curriculum, reporting, and analyzing program data. The CHW Program Manager operates within a public health framework emphasizing prevention, health equity, and community-based care integration. This role is accountable for full population management across the CHW-supported population, including engagement, outcomes, and integration with care management and quality. Under the department HE Director's leadership, the Community Health Worker Program Manager will serve as the primary contact to the specific vendor/partner interacting extensively with internal and external stakeholders, including members. The Community Health Worker Program Manager will also actively develop policies and procedures and execute the specific required tasks for the manager is responsible for the creation and delivery of presentations to internal and external stakeholders surrounding the program contractual compliance, status of patient care, successes in operations and opportunities for improvements. The role ensures alignment with existing chronic disease infrastructure and positions CHW services as a core component of the overall clinical and population health strategy.
