Requirements

High School Diploma or GED plus 1-3 years of experience with CHW work.
Strong organizational skills, attention to detail, and ability to manage multiple tasks.
Strong written and verbal communication skills.
Ability to work both independently and collaboratively with diverse team members.
Commitment to cultural humility and ability to work with diverse, underserved communities.
Familiarity with community outreach, data tracking systems, and case documentation.

Nice To Haves

Community Health Worker (CHW) certification or equivalent training is preferred.
Strong data entry and data management skills, including familiarity with Excel or Google Sheets Preferred.
Experience working or volunteering in a community health center, social service program, or community-based organization.

Responsibilities

Support the day-to-day operations of the CHW Program, including scheduling meetings, preparing materials, and following up on action items.
Assist the Vice President of Programs and Coordinator with coordination tasks to ensure timely execution of program activities.
Supporting with CHW Certification applications.
Support the logistics of CHW training, including preparing materials and tracking participation.
Assist with CHW orientation and ongoing development activities to ensure CHWs receive comprehensive and effective training.
Support in maintaining accurate training records and certifications for CHWs.
Support CHWs in the collection of outreach data and documentation of services and activities.
Maintain and update program databases and spreadsheets to accurately track client services, referrals, and program activities.
Generate routine data reports for internal use, funder requirements, and quality improvement efforts.
Maintain confidentiality and comply with all data security and privacy requirements.
Coordinate and participate in community engagement activities, including events, workshops, and tabling efforts.
Support and actively participate with CHWs in engaging community members to assess and address needs related to housing, utilities, health, nutrition, and employment.
Provide logistical and on-site support for community events and outreach activities.
Function in a Community Health Worker role as needed, including outreach, basic navigation, and community engagement, under the guidance of the Program Coordinator or VP of Programs.
Provide logistic support for community events, workshops, and other outreach activities to promote health initiatives and engage residents.
Perform additional duties assigned to support the overall goals and needs of the CHW Program.
