About The Position

Requirements

  • High School Diploma or GED plus 1-3 years of experience with CHW work.
  • Strong organizational skills, attention to detail, and ability to manage multiple tasks.
  • Strong written and verbal communication skills.
  • Ability to work both independently and collaboratively with diverse team members.
  • Commitment to cultural humility and ability to work with diverse, underserved communities.
  • Familiarity with community outreach, data tracking systems, and case documentation.

Nice To Haves

  • Community Health Worker (CHW) certification or equivalent training is preferred.
  • Strong data entry and data management skills, including familiarity with Excel or Google Sheets Preferred.
  • Experience working or volunteering in a community health center, social service program, or community-based organization.

Responsibilities

  • Support the day-to-day operations of the CHW Program, including scheduling meetings, preparing materials, and following up on action items.
  • Assist the Vice President of Programs and Coordinator with coordination tasks to ensure timely execution of program activities.
  • Supporting with CHW Certification applications.
  • Support the logistics of CHW training, including preparing materials and tracking participation.
  • Assist with CHW orientation and ongoing development activities to ensure CHWs receive comprehensive and effective training.
  • Support in maintaining accurate training records and certifications for CHWs.
  • Support CHWs in the collection of outreach data and documentation of services and activities.
  • Maintain and update program databases and spreadsheets to accurately track client services, referrals, and program activities.
  • Generate routine data reports for internal use, funder requirements, and quality improvement efforts.
  • Maintain confidentiality and comply with all data security and privacy requirements.
  • Coordinate and participate in community engagement activities, including events, workshops, and tabling efforts.
  • Support and actively participate with CHWs in engaging community members to assess and address needs related to housing, utilities, health, nutrition, and employment.
  • Provide logistical and on-site support for community events and outreach activities.
  • Function in a Community Health Worker role as needed, including outreach, basic navigation, and community engagement, under the guidance of the Program Coordinator or VP of Programs.
  • Provide logistic support for community events, workshops, and other outreach activities to promote health initiatives and engage residents.
  • Perform additional duties assigned to support the overall goals and needs of the CHW Program.
