About The Position

New Kensington Community Development Corporation (NKCDC) is a community development corporation committed to sustainable development in housing opportunities, economic development, vacant land strategies, community engagement, health and wellness, and real estate development. Requirements

  • High School Diploma or GED plus 1-3 years of experience with CHW work.
  • Ability to effectively communicate and engage with diverse audiences in public settings.
  • Basic computing knowledge, including proficiency in Word, Excel, and Outlook.
  • Strong interpersonal skills, communication skills, flexibility, and attention to detail.
  • Commitment to cultural humility and ability to work with diverse, underserved communities.

Nice To Haves

  • Community Health Worker (CHW) certification or equivalent training is preferred.
  • Bachelor's Degree in Social work or Human Services preferred.
  • Previous case management experience preferred.
  • Previous experience in community outreach, engagement, or program coordination is preferred.
  • Strong organizational and time management skills, with the ability to manage multiple tasks and prioritize effectively.
  • Familiarity with the Kensington neighborhood is a plus.
  • Bilingual (Spanish and English) preferred.
  • Flexibility to work off-site, and to work evenings, and weekends occasionally as needed.

Responsibilities

  • Oversee the day-to-day operations of the CHW program, including the coordination and support of CHW training, orientation, and ongoing development.
  • Adhere to the Care Management Model.
  • Ensure that CHWs are effectively engaged in community outreach and that they adhere to program guidelines and goals.
  • Coordinate training sessions for new CHW cohorts, including preparation, orientation, and support throughout the training process.
  • Assist instructors with training logistics and content, ensuring that CHWs receive comprehensive and effective training.
  • Facilitate CHWs in engaging with community members to assess and address needs related to housing, utilities, health, nutrition, employment, and other services.
  • Support CHWs in conducting surveys, collecting data, and documenting outreach activities.
  • Support the Development of relationships with community organizations, city agencies, businesses, and schools to enhance resources and support for the CHW program.
  • Strengthen connections between NKCDC, external partners, and other organizations to create resources for client participants.
  • Provide administrative support and coordination for the CHW program, including documentation, case management reviews, activity tracking, and report preparation.
  • Maintain accurate records of training, outreach efforts, case reviews, and program impact.
  • Support grant reporting requirements.
  • CHW certifications; coordinate certification requirements, track certification completions, submit certification applications.
  • Organize and plan community events, workshops, and other outreach activities to promote health initiatives and engage residents.
  • Coordinate ongoing professional development activities for CHWs, including access to training, workshops, and career advancement opportunities.
  • Serve as a role model for CHWs, providing support for their professional growth.
  • Perform additional tasks as assigned to support the overall goals and needs of the CHW program.
