The Connected Care Community Health Worker plays an important role in helping older adults and their families access the information, resources, and coordinated care they need to achieve their individual care goals. Working closely with patients, their families, primary care providers, and behavioral health providers, this position helps ensure that what matters most to each patient is understood and communicated across the care team. In this role, the Connected Care Community Health Worker supports patients with important aspects of their health and well-being, including completing Advance Directives, reconciling medications, identifying and addressing fall hazards in the home, and connecting patients and families with community resources and services. This position is part of an innovative pilot program designed to strengthen care and support for frail older adults. The Connected Care Community Health Worker will partner closely with primary care providers and the Connected Care team to implement, test, and refine pilot protocols while supporting an integrated, multidisciplinary approach to patient care.
