Requirements

  • High school diploma or equivalent
  • Bilingual in English and Spanish
  • Community Health Worker (CHW) certification or willingness to obtain certification within 1 year of hire required
  • Required to complete the federal training to obtain Consumer Assistance Certification within one (1) month of hire and maintain certification through annual recertification to assist consumers seeking health insurance coverage options through a Federally Facilitated Marketplace
  • Required to complete online training to obtain Navigator certification through the Texas Community Partner Program within one (1) month of hire and maintain certification through required recertification to enroll patients in My Texas Benefits
  • Required to complete online Indigent Healthcare Solutions training for the County Indigent Health Care Program (CIHCP) within one (1) month of hire
  • Experience working with populations served by public health, community health centers, or similar underserved populations
  • Knowledge of medical terminology
  • Knowledge of adult learning principles and interviewing and counseling techniques
  • Knowledge of case management processes and community resources
  • Experience working with individuals living with chronic diseases
  • Ability to facilitate and/or co-facilitate group education or intervention activities
  • Strong customer service, communication, and computer skills (MS Office)
  • Ability to work independently while managing multiple priorities
  • Ability to work evenings and weekends as needed
  • Valid Texas driver's license (or ability to obtain) and reliable transportation
  • Must comply with GCHD Immunization Policy and ICS training requirements
  • Must pass a criminal background check and drug/alcohol screening
  • Equivalent education and experience may be considered

Nice To Haves

  • 6 months or more of Community Health Worker experience
  • Knowledge of public assistance programs
  • Program development and assessment experience
  • Stanford Lay Leader training
  • Experience with patient outreach, care coordination, health education, or enrollment assistance for Medicaid, Medicare, Marketplace plans, or other public assistance programs

Responsibilities

  • Educate patients on chronic disease management, provider care plans, and available healthcare resources
  • Assist patients with applications, renewals, and enrollment for Texas Medicaid, Medicare, Health Insurance Marketplace plans, and the County Indigent Health Care Program (CIHCP)
  • Schedule follow-up appointments before and after hospitalizations to promote continuity of care
  • Contact patients from emergency department discharge reports and hospital referral lists to coordinate follow-up care
  • Connect patients with healthcare, social service, and community resources
  • Conduct patient follow-up by phone, home visits, and other community settings as appropriate
  • Plan, coordinate, and participate in community outreach and health education events
  • Facilitate communication and coordinate services between patients, providers, case managers, and community partners
  • Maintain accurate documentation while adhering to HIPAA and organizational policies
  • Build supportive relationships that encourage patients to actively participate in managing their health
  • Collaborate with internal departments and outside agencies to improve access to care
  • Respond to public health emergencies and support organizational initiatives
  • Carry out the mission, vision, and values of the organization
  • Perform other duties as assigned
