The Community Health Worker-Behavioral Health will accept patient referrals and warm hand offs, both in-person and virtually, from LifeLong providers and staff. This role involves contacting, screening, and triaging patients using standardized tools, diagnostics, and structured interviews to identify needs, assess program eligibility, determine patient interest in services, and maintain a referral waitlist. The CHW will develop trusting relationships with patients, promoting patient-centered care and supporting them in achieving their health goals. This includes facilitating connections to various health and social services, both internal and external to LifeLong, assisting with scheduling appointments, health insurance, access to medications, and transportation. The role requires tracking patient attendance, following up on no-shows, and conducting appointment reminder calls. The CHW will also provide schedules to optimize patient access and productivity, collect and enter data accurately into the Electronic Health Record and other systems, and generate reports. They will provide patients with educational materials and resource lists, track patient progress using data reports, and participate in care team meetings and case conferences. Maintaining up-to-date information on community resources and promoting timely, coordinated internal communications among care team members are key responsibilities. The CHW will involve family members as necessary and promote diversity, equity, and inclusion. The role requires conducting oneself professionally in external settings and demonstrating a proactive, creative, and mature attitude with a commitment to working directly with low-income individuals from diverse backgrounds.
