About The Position

This is a part-time position for a Community Health Worker Apprentice in Behavioral Health. The role involves accepting patient referrals, building trusting relationships, and connecting patients to health and social services. The apprentice will track patient progress, collect data, and participate in care team meetings. The position requires working 32 hours per week onsite at the health center until graduation from the apprenticeship program. The apprentice must maintain enrollment and good standing in the CHW Apprenticeship program, including completion of all required classes and trainings. The role emphasizes working directly with low-income individuals from diverse backgrounds in a supportive manner and promoting diversity, equity, and inclusion.

Responsibilities

  • Accept patient referrals and warm hand offs, both in-person and virtually, from LifeLong providers and staff.
  • Contact, screens, and triage patients, using standardized tools, diagnostics and structured interviews as appropriate and required by program, to identify needs, assess eligibility for a variety of programs, determine patient interest in services, and maintain referral waitlist if needed.
  • Develop consistent, trusting relationships with patients, promoting patient centered care and support for patients to achieve their health goals.
  • Facilitates connections to a variety of health and social services both within and external to LifeLong, including referrals to community resources and assisting with scheduling appointments, health insurance, access to medications and transportation.
  • Track patient attendance at appointments, follow up on no-shows to support and improve continuity of treatment and conduct appointment reminder calls per program protocol.
  • Provides schedules, as needed, in order to optimize patient access and productivity.
  • Collect and enter data into the Electronic Health Record and other required data systems in a timely and accurate manner, generating reports as required to support workflows.
  • Provide patients with accessible educational and other materials including resource lists, releases of information etc.
  • Track patients’ progress using data reports and other tools available through LifeLong data systems.
  • Participate with care team in developing guidelines and workflows for services.
  • Attend team meetings and case conferences; CHW may facilitate, take minutes, and follow up with action steps.
  • Maintain up to date information on health, educational, behavioral health and social service agencies in order to facilitate referrals and coordinate patient care.
  • Promote timely and coordinated internal communications among patients, medical, behavioral health and dental providers, case managers, and others involved in the patient’s care provision to inform and collaborate on care needs as they arise.
  • Attend external meetings as needed to ensure care coordination.
  • Involve family members/significant others as necessary to increase patient participation and improve patient well-being.
  • Promote diversity, equity and inclusion in support of patients and staff.
