The Community Health & Resource Navigator is a patient-facing role focused on building relationships and helping patients and families understand their options and take practical next steps to access health care, social services, and community resources. Drawing on experience in social services, human services, community programs, advocacy, or a related field, the Navigator listens to patients’ needs, explains available options in plain language, and supports their confidence in navigating systems. The role emphasizes personal connection, patient choice, and self-advocacy. The Navigator works directly with patients, generally in person when possible, helps connect them with appropriate services, and follows up as appropriate. The role also includes supporting a welcoming patient experience, communicating recurring barriers to a supervisor or appropriate team, partnering with other staff when needed, documenting interactions and follow-up needs, and maintaining confidentiality. The work environment is usually moderate in noise level. The listed tasks and responsibilities are not exhaustive, and equivalent experience, skills, or education may be considered.
