About The Position
This is a part-time position budgeted for 20 hours per week. The anticipated hiring range for this position will be $41,176 - $52,795.50, based on the candidate’s qualifications and experience. The hourly rate range is $39.59 - $50.76. Montgomery County is located adjacent to Washington, D.C., and with over one million residents, is the most populous County in the State of Maryland. DHHS Public Health Services Division is currently seeking a compassionate and dedicated Community Health Nurse who is passionate about maternal and child health. As a Community Health Nurse, you will become part of a supportive, multidisciplinary team that works together to empower families through education, advocacy, care coordination, and home-based nursing services in Montgomery County.
Requirements
- Graduation from a National League for Nursing accredited college or university or an equivalent program as determined by the Maryland Board of Examiners with a Bachelor's Degree in Nursing.
- Two (2) years of full-time equivalent post-licensure experience as a Registered Nurse, one year of which must have been in the nursing area of Maternal and Child Health.
- Possession of a current license to practice as a Registered Nurse in the Maryland State, of Maryland issued by the Maryland Board of Examiners is required at the time of appointment. If the license is temporary, a regular Maryland RN license must be obtained within ninety (90) days of appointment.
- Possession and maintenance at all times of a valid driver's licensure from your state of residence.
Nice To Haves
- Experience in Maternal Child Health/Public Health setting such as a health center, home visiting program, or in a specialty area to include prenatal care, obstetrical care, newborn care, reproductive health, pediatrics, and family-based case management.
- Experience working with a diverse, high-risk, African American community.
- Experience in Maternal Child health promotion & breastfeeding counseling.
- Experience in providing home visiting services with pregnant women and children under the age of two.
- Knowledgeable of the community-based resources in Montgomery County serving high risk populations.
- Experience using an Electronic Health Record
- Experience coordinating services with community partners, Primary medical doctors, and community organizations
Responsibilities
- Provide home-based and community-based nurse case management services to high-risk African American pregnant and postpartum women and their newborns.
- Conduct comprehensive initial and ongoing assessments of pregnant women, postpartum individuals, infants, and adolescents to identify medical, psychosocial, environmental, and educational needs.
- Develop individualized care plans and implement evidence-based interventions to address identified health and social needs.
- Monitor client progress toward established goals and update care plans as needed.
- Make referrals and coordinate services with healthcare providers, behavioral health providers, social service agencies, and community resources.
- Collaborating with program staff and community partners to showcase program successes and outcomes.
