Community Health Navigator

Reema HealthKansas City, KS
$50,000 - $56,000Remote

About The Position

This role focuses on outreach and engagement to enroll members in Reema's care model. The Community Health Navigator will build and manage a caseload of 80-120 members, providing coverage for other Guides during absences or workload spikes. The position involves assessing member needs, including social determinants of health, to build member-centered care plans. A key aspect of the role is supporting members toward their goals by establishing trusting relationships, helping them understand health activities, and guiding them through barriers to care with visible progress. The Navigator will coordinate referrals, support appointment scheduling and attendance, and independently organize their workload to meet monthly goals and daily metrics. Proactive engagement with members through various channels, connecting them to community and health resources (including transportation), and maintaining professional boundaries and confidentiality are essential. Thorough and timely documentation in the Reema App, including keeping care plans current, and compliance with HIPAA are required. The role also involves representing Reema well by working independently and collaboratively in a fast-paced startup environment, seeing change as an opportunity, and serving members with dignity and respect. Job responsibilities may grow and change as business needs evolve.
