Partnering closely with care teams and community organizations, you’ll help improve access, coordination, and overall well-being. Establishes relationships with a wide range of constituencies in a diverse community and provides feedback. Works closely with healthcare teams and other community-based organizations through activities related to identified health priorities identified.

Requirements

  • High School diploma/GED or 10 years of work experience
  • One year experience working with people from diverse backgrounds or community programs
  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
  • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
  • Frequent use of hearing and speech to share information through oral communication.
  • Ability to hear alarms, malfunctioning machinery, etc.
  • Frequent keyboard use/data entry.
  • Occasional bending, stooping, kneeling, squatting, twisting and gripping.
  • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
  • Rare climbing.

Responsibilities

  • Promotes wellness by providing culturally appropriate health information and education to clients and providers.
  • Assists clients in accessing health related services by obtaining a primary care provider and overcoming barriers to obtaining needed care and social services.
  • Increases self-efficacy to achieve wellness and helps community members in accessing resources.
  • Schedules appointments and assists with completion of application for programs for which they may be eligible.
  • Provides direct services including lifestyle and survey assessments, health education, and basic health screenings.
  • Documents all activities as detailed by direct supervisors.
  • Builds and maintains positive working relationships with the community members, healthcare teams, and community-based organizations.
  • Works to expand knowledge and understanding of community resources, services and programs.
  • Performs other duties as assigned.
