About The Position

The Community Habilitation Coordinator plays an integral role in supporting individuals receiving Community Habilitation services. This position provides oversight, guidance, and person-centered planning for individuals on an assigned caseload while supporting and supervising direct support professionals. Coordinators ensure high-quality service delivery, regulatory compliance, and a positive, inclusive environment that reflects Kelberman’s mission, values, and commitment to excellence.

Requirements

  • Proficiency with Microsoft Office Suite or related software.
  • Demonstrated strengths in teamwork, flexibility, problem-solving, and customer service.

  • Bachelor's degree in human services or related fields preferred.
  • 1–3 years of experience supporting individuals with developmental disabilities required.
  • OPWDD QIDP certification preferred.
  • Valid NYS Driver’s License required; travel is necessary.
  • Must complete fingerprinting and background checks per OPWDD regulations.

Responsibilities

  • Promote the safety, well-being, and independence of individuals receiving services.
  • Provide weekly direct support hours and implement individualized goals and supports.
  • Deliver direct support services as needed to ensure continuity of care.
  • Develop, implement, and document Staff Action Plans (SAPs) aligned with Life Plan outcomes.
  • Maintain timely, accurate, and complete documentation and records.
  • Support individuals with personal care, self-care, and other needs they cannot complete independently.
  • Provide daily oversight and guidance to direct support staff in home and community settings.
  • Communicate expectations clearly and foster a supportive, collaborative team environment.
  • Offer coaching, feedback, and performance support to enhance staff growth and effectiveness, including addressing disciplinary needs in conjunction with supervisor and human resources if warranted.
  • Complete 90-day and annual performance evaluations for direct support staff.
  • Assist with onboarding, orientation, and ongoing training for new and existing employees.

  • Represent Kelberman in a professional manner in all interactions
  • Participate in treatment team meetings, training, and collaborative planning.
  • Maintain strong communication with individuals, families, staff, community partners, and internal departments.
  • Ensure compliance with OPWDD regulations, agency policies, and quality standards.
  • Contribute to a work environment that promotes respect, teamwork, and alignment with the agency's mission.
  • Participate in committees, training, and other team activities as assigned.
  • Perform additional duties as needed to support program success.
