This role involves utilizing evidenced-based models and practices to engage residents in services and community activities, providing case management and intensive life skills to individuals and client groups. The specialist will develop and maintain a community resources listing, conduct follow-up and coordination with other social agencies, and assist consumers with their individualized recovery service plans. Accurate record-keeping, service advocacy, and coordination of human service programs are key responsibilities. The role also includes conducting home visits, transporting participants, and adhering to all policies and confidentiality regulations.
