About The Position

Care Alliance Health Center is seeking a Community Engagement – School-Based to support health programs within schools and youth-serving organizations. This role helps connect students, families, educators, and community partners to healthcare services, resources, and support. At Care Alliance, our mission is to provide exceptional, accessible, and compassionate healthcare for all. As a School-Based Community Health Worker, you will play an important role in improving access to care, supporting families, and promoting health education in school and community settings.

Requirements

  • Bachelor’s degree in public health, Social Work, Community Development, or a related field
  • 2–3 years of experience in community engagement, outreach, or program coordination
  • Strong communication and organizational skills
  • Compassionate and dependable professional who enjoys working with youth, families, schools, and community partners
  • Strong communicator, highly organized, and able to build trust with diverse communities

Nice To Haves

  • Community Health Worker certification or willingness to obtain
  • Experience working in school-based or youth-serving environments
  • Experience with electronic health records and patient access workflows

Responsibilities

  • Collaborate with school partners and internal teams to identify student and family needs
  • Plan and coordinate school-based programs, health fairs, screenings, and educational events
  • Manage logistics for on-site activities, including setup, materials, and workflow coordination
  • Provide health education and support to students and families
  • Assist with patient registration, intake, and demographic verification
  • Schedule appointments and connect students and families to medical, dental, and behavioral health services
  • Conduct assessments to identify social needs and connect families to resources
  • Support care coordination, follow-up, and continuity of care
  • Build and maintain relationships with school staff, families, and community partners
  • Promote Care Alliance services through outreach, communication, and engagement activities
  • Represent Care Alliance at school and community events
  • Document outreach, registration, referrals, and follow-up activities in the electronic health record
  • Track participation, outcomes, and engagement metrics to support reporting and program improvement
