About The Position

Chrysalis Center is seeking a passionate and empathetic Community Case Manager to join their HEARRT House program. This role is ideal for individuals who are driven to make a difference in people's lives, possess strong problem-solving skills, and have a heart for helping others. The Community Case Manager will serve as a hero of hope, utilizing their knowledge, empathy, and organizational skills to connect clients with necessary resources for success.

Requirements

  • 1 to 3 years of experience in case management services with veterans, mental health, substance abuse, HIV/AIDS, or homeless clients.
  • Compassionate and patient listener who believes in people’s potential.
  • Ability to juggle multiple tasks.
  • Problem-solver who thrives in fast-paced, dynamic environments.
  • Organized, detail-oriented, and able to see projects through to completion.
  • Previous experience as a Case Manager or in social services is a huge plus.
  • Bachelor’s degree in Social Work, Psychology, or a related field preferred (life-learned experts are also encouraged to apply).

Responsibilities

  • Serve as the first point of contact and build trust with clients facing complex life situations.
  • Develop individualized service plans to guide clients toward achieving their goals.
  • Connect clients with community resources, services, and support networks.
  • Advocate on behalf of clients for housing, healthcare, financial assistance, and more.
  • Track and document progress, celebrating victories big and small.
  • Collaborate with a passionate team of colleagues and partners to create lasting impact.
