About The Position

Marrakech is seeking a dedicated Case Manager to support young adults and adults living independently in the community. As a key member of the treatment and recovery team, this role provides case management, life skills training, care coordination, and supervision of support staff to help individuals achieve independence, wellness, and successful community integration. Marrakech provides person-centered support and advocacy that enhances quality of life and promotes independence for individuals with and without disabilities always with dignity, respect, and meaningful connection at the center. For more than 50 years, Marrakech has supported over 1,000 individuals across Connecticut each year, helping people of all abilities build confidence, independence, and strong community connections. Requirements

  • Bachelor’s Degree in a related field (with relevant classroom and internship experience).
  • Demonstrable, relevant clinical experience in a residential or community setting or an Associates’ degree with three years of relevant clinical experience carrying out treatment plans in residential or other community settings.
  • Valid driver's license and reliable transportation

Nice To Haves

  • Master’s degree and supervisory experience in related field is preferred.
  • Ability to coordinate services and supervise direct support staff
  • Knowledge of person-centered planning, independent living skills instruction, and trauma-informed care preferred

Responsibilities

  • Manage a caseload and develop individualized service plans in collaboration with clinical providers and community partners
  • Coordinate healthcare, appointments, transportation, medication monitoring, and other support services
  • Teach and support independent living skills, including budgeting, cooking, housekeeping, personal care, decision-making, and community engagement
  • Supervise and support direct care staff delivering daily living skills instruction
  • Implement behavioral support plans, treatment recommendations, and trauma-informed care practices
  • Complete assessments, maintain accurate documentation, and ensure compliance with program and DMHAS requirements
  • Monitor individual safety and well-being while providing meaningful community experiences
  • Participate in team meetings, provide case updates, and collaborate with families, providers, and community agencies
  • Respond to emergencies, follow on-call procedures, and implement CPR, First Aid, and crisis intervention techniques as required
