About The Position

The Domestic Violence Coordinated Entry (CE) Community Case Manager provides mobile, survivor-centered outreach, assessment, and navigation services for individuals and families experiencing homelessness due to domestic violence, dating violence, sexual assault, and/or stalking. This position ensures survivors have safe, equitable access to the Coordinated Entry System while maintaining strict confidentiality and compliance with Violence Against Women Act protections. This role prioritizes unsheltered and highly vulnerable survivors, utilizing trauma-informed and safety-focused practices to connect participants to housing and supportive services. The Community Case Manager position is funded wholly through HUD’s CE DV Bonus grant. The Community Case Manager will dedicate 100% of their time working with unsheltered individuals and families who are experiencing homelessness due to domestic violence, dating violence, sexual assault, and/or stalking. One80 Place ends and prevents homelessness throughout South Carolina with offices located in Charleston and Columbia. This position will be based in Charleston.

Requirements

  • Bachelor’s degree in social work, human services, or related field (or equivalent experience).
  • 2+ years experience working with survivors of domestic violence, sexual assault, stalking, or human trafficking.
  • Strong understanding of trauma-informed care, safety planning, and survivor advocacy.
  • Knowledge of crisis intervention and service coordination.
  • Ability to maintain strict confidentiality and ethical standards.
  • Ability to deal effectively with conflict and crisis in a calm manner to foster a positive resolution.
  • Excellent computer skills including basic office programs and the ability to learn and utilize the HMIS database.
  • Valid driver's license and reliable transportation.
  • Ability to travel throughout the 7-county service area of the Low Country Continuum of Care in a personal vehicle or company-provided vehicle.

Responsibilities

  • Provide trauma-informed, survivor-centered outreach in community and nontraditional settings to build trust, ensure safety and confidentiality, and connect survivors—especially those not engaged with traditional systems—to supportive services.
  • Act as a dedicated DV access point for Coordinated Entry by conducting trauma-informed, safety-focused assessments, protecting survivor confidentiality, and clearly explaining the process, resources, and participant rights.
  • Support safe and equitable housing prioritization and referrals by using CoC-approved tools, participating in case conferencing, and helping survivors access appropriate housing resources while guiding them through placement with ongoing safety planning.
  • Deliver community based, safety-focused housing case management that includes ongoing safety planning and crisis support, assistance with documentation and benefits, survivor advocacy across systems, and connections to healthcare, legal, and community resources.
  • Maintain accurate, secure DV data in a HMIS comparable database while ensuring compliance with VAWA, FVPSA, and HUD CoC requirements, protecting survivor confidentiality, and using only de-identified information for Coordinated Entry participation and reporting.
  • Collaborate with Coordinated Entry and community partners to strengthen survivor access to services, participate in confidential system coordination, and support survivor-centered improvements across the CES.
  • Ensure timely and accurate HMIS data entry and maintenance and complete annual and update training requirements for HMIS.
  • Attend and participate in internal staff meetings and with partner agencies to effectively coordinate service delivery for clients engaged through outreach.
  • Participates in agency events such as the annual Homeless Point in Time Count.
  • Works in collaboration with all other One80 Place staff to facilitate a team environment.
  • Participates in One80 Place training as required.
  • Actively participates in monthly staff and team meetings and commits to group decisions.
  • Substitutes for other staff when the need arises.
  • Performs other relevant duties assigned by supervisor or other One80 Place management staff.
