Community Care Coordinator (Home Health & Hospice)

As a Hospice Community Liaison, your network becomes more valuable because you can support referral partners with hospice, home health, pharmacy services, mobile providers, and more. Requirements
- 1+ years of hospice marketing experience or proven healthcare sales experience
- Established referral relationships within the community
- Knowledge of Medicare hospice guidelines and compliance standards
- Exceptional communication and relationship-building skills
- Ability to work independently and achieve goals
- Valid driver's license, reliable transportation, and auto insurance
Responsibilities
- Develop and maintain referral relationships with physicians, hospitals, SNFs, ALFs, case managers, and community organizations
- Conduct face-to-face outreach and educational presentations
- Represent Billet Health at community events and networking opportunities
- Meet with patients and families to educate them on hospice services and benefits
- Track activities, leads, and conversion outcomes
- Identify market opportunities and support growth initiatives
