About The Position

The Community-Based Registered Nurse conducts scheduled, non-clinical nursing assessments in clients’ homes. The role uses conversation and observation to understand clients’ health histories, functional needs, daily routines, and personal goals, then develops practical individualized care plans for non-medical in-home caregivers to follow. The position focuses on assessment, communication, documentation, care coordination, and relationship-based support—not direct patient care, clinical procedures, or personal care. The nurse also serves as a clinical resource to the care team and collaborates with the office team to support care delivery.

Requirements

• Active Registered Nurse (RN) license.
• Ability to complete thorough verbal and observational assessments.
• Strong documentation and care-planning skills.
• Comfortable working independently in the community.
• Professional, compassionate communication style with clients, families, and caregivers.

Responsibilities

• Travel to clients’ homes for scheduled, non-clinical nursing assessments.
• Conduct verbal and observational assessments with clients and family members.
• Review medical history, diagnoses, medications, functional abilities, and daily routines.
• Identify risks, support needs, and appropriate non-medical services.
• Develop and update individualized care plans for in-home caregivers to follow.
• Serve as a clinical resource to the care team regarding clients’ plans of care.
• Maintain accurate and timely documentation of assessments and care plans.
• Collaborate with the office team to support care delivery.
