Community-Based Registered Nurse - Home Care
About The Position
This Registered Nurse role uses clinical knowledge to conduct non-clinical in-home assessments and develop practical care plans for non-medical in-home caregivers. The work centers on conversation, observation, documentation, care planning, and relationship-based support rather than direct or physical patient care. The nurse reviews clients’ health backgrounds, functional needs, and personal goals to create realistic, supportive plans. The position involves traveling to clients’ homes for scheduled assessment visits and collaborating with the office team to support care delivery. It has no hands-on care, clinical procedures, lifting, transfers, bathing, or other personal care responsibilities.
Requirements
- Active Registered Nurse (RN) license.
- Ability to complete thorough verbal and observational assessments.
- Strong documentation and care-planning skills.
- Comfortable working independently in the community.
- Professional, compassionate communication style with clients, families, and caregivers.
Responsibilities
- Travel to clients’ homes to complete scheduled, non-clinical nursing assessments.
- Conduct verbal and observational assessments with clients and family members.
- Review medical history, diagnoses, medications, functional abilities, and daily routines.
- Identify risks, support needs, and appropriate non-medical services.
- Develop and update individualized care plans for in-home caregivers to follow.
- Serve as a clinical resource to the care team regarding the client’s plan of care.
- Maintain accurate and timely documentation of assessments and care plans.
