The Registered Nurse (RN) is a vital member of the Assertive Community Treatment (ACT) team, delivering recovery-oriented, person-centered care to individuals with serious mental illness in the community. This role blends clinical skill, crisis response, and collaboration to support clients in achieving stability, independence, and improved quality of life. The ACT team is a multidisciplinary group dedicated to supporting adults with serious mental illness (SMI) through community-based services, including therapy, medication management, and resource linkage. Services are primarily delivered in the community, with clients receiving at least three home visits per week. The RN contributes to a holistic approach to care, helping clients integrate into society while ensuring their basic needs and treatment goals are met. Frequent travel (50-90%) is required, and personal vehicle use for client transportation may be necessary.
