About The Position

Hospice and Palliative Care Buffalo is hiring Registered Nurses (RNs) to join our growing clinical team. We currently have opportunities in home care, facility-based care, community admissions, and on-call nursing. Our nurses provide compassionate, patient centered hospice and palliative care that helps patients maintain their comfort, dignity, independence, and quality of life. Requirements

• Graduate of an accredited school of nursing.
• Current Registered Nurse (RN) license in the State of New York.
• Valid driver's license and access to a vehicle for positions requiring community travel.

Nice To Haves

• One year of professional nursing experience preferred for most Registered Nurse opportunities.
• Two years of professional nursing experience preferred for the Community Admissions Registered Nurse position.
• Certification in Hospice and Palliative Care Nursing preferred for applicable positions.

Responsibilities

• Provide compassionate, individualized nursing care to hospice and palliative care patients.
• Perform comprehensive assessments addressing physical, psychosocial, environmental, safety, emotional, and spiritual needs.
• Assess and manage pain and other symptoms associated with serious and terminal illness.
• Develop, initiate, and update individualized plans of care based on patient needs.
• Recognize changes in patient condition and provide timely and appropriate clinical interventions.
• Educate and support patients, families, and caregivers.
• Collaborate with physicians and interdisciplinary team members to coordinate patient care.
• Help patients and families understand hospice services and appropriate levels of care when applicable.
• Maintain accurate and timely electronic documentation.
• Follow established clinical, patient safety, privacy, and infection-control standards.
• Travel to patient homes and/or healthcare facilities as required by the position.
