About The Position

The NorWest Community Health Centres, a community-based, non-profit organization funded by Ontario Health, is part of a network of provincial community health centres. The organization emphasizes access to primary health care and prevention programs and services. They are seeking three full-time equivalent (FTE) temporary contract Community Health Workers for an 8-week period. These workers will be valuable members of the Community Health Centre team, providing support to clients to improve health and well-being, reducing social isolation through program delivery, and connecting clients with long-term community services. The Community Health Worker will collaborate closely with Allied Health Programs, the inter-professional care team, and community partners to assist clients in meeting their needs and achieving goals related to the determinants of health.

Requirements

  • Currently enrolled as a student and interest in social work or community health services
  • Ability to support clients given a social prescription for tasks
  • Ability to function as part of a multidisciplinary team in a community health environment
  • Ability to organize opportunities for community members
  • Promote opportunities for volunteers and community members
  • Must be between 15 and 30 years of age at the beginning of the employment period
  • Canadian citizen, permanent resident, or person to whom refugee protection has been conferred under the Immigration and Refugee Protection Act for the duration of the employment
  • Have a valid Social Insurance Number at the start of employment and be legally entitled to work in Canada in accordance with relevant provincial or territorial legislation and regulation
