About The Position

VHA Home HealthCare (VHA) is seeking a Community Access Physiotherapist to join their UHN@Home Program. This full-time role involves providing physiotherapy consultation, assessment, and short-term intervention to patients transitioning from hospital to home within the Toronto Central region as part of the UHN Transitions Program. The position supports a structured, time-limited model of care (16 weeks) focused on stabilization, functional improvement, and facilitating safe discharge and reintegration into the community. The Physiotherapist is expected to complete initial patient visits within 48 hours of hospital discharge and will collaborate with a Rehabilitation Assistant. The role emphasizes interdisciplinary collaboration and connecting patients to long-term community supports.

Requirements

  • Masters degree in Physiotherapy from a recognized school of physiotherapy.
  • Registered, in good standing, with the College of Physiotherapists of Ontario.
  • Minimum of 1 year clinical experience, preferably in the community.
  • Specialized courses to support areas of clinical expertise.
  • Clinical expertise to provide a variety of physiotherapy interventions.
  • Knowledge of community care requirements and resources available.
  • Excellent consultation and teaching skills, and utilizes an evidence base to clinical practice.
  • Familiar with the routine use of valid and reliable outcome measures to support clinical interventions.

Responsibilities

  • Assesses clients using interviews, standardized and informal assessment techniques in accordance with professional practice guidelines and standards established by VHA.
  • Implements client interventions based on assessment information, client input and in conjunction with the client’s integrated care plan.
  • Ensures that services provided are best practice, evidence-based and evaluated through the routine use of valid and reliable outcome measured.
  • Attends client related meetings to report on assessment findings and discuss plans for intervention, progress reports and discharge as required.
  • Ensures coordinated delivery of care through ongoing communication with all members of the client’s team regarding plans and schedules for interventions.
  • Documents all assessment findings, treatment plans, progress and discharge summaries of interventions as required by the profession, referral source and the organization.
  • Prepare statistical records of client care and related activities as required by VHA, the Ministry of Health and other agencies.
  • Adheres to the code of ethics of the profession, in maintaining standards of practice for the profession as defined by the Ontario College of Physiotherapists.
