About The Position

As a Home and Community Based Services Aide with Heartlite Hospice, part of LHC Group and the Optum family of businesses, you will support clients in remaining in their own homes by assisting with personal hygiene and household functions. The role involves personal care and daily living support, household tasks, client monitoring and documentation, and maintaining a safe environment. You must be able to function in any home situation regardless of the client’s age, race, creed, color, sex, disability, or financial condition. The hourly pay range is $17.50 to $19.50 based on full-time employment; pay is determined by factors such as local labor markets, education, work experience, and certifications. Requirements

Current driver’s license, vehicle insurance, and reliable transportation or access to public transportation.

Nice To Haves

Current CPR certification or ability to complete it within 90 days of hire.
At least 6 months of home care experience.
Ability to work flexible hours and independently.

Responsibilities

Provide personal care and assist with daily living activities, including bathing, grooming, dressing, ambulation, and medication reminders.
Support household tasks and meal preparation, and accompany clients to appointments or errands as needed.
Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use the EVV system.
Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
Function in any home situation regardless of the client’s age, race, creed, color, sex, disability, or financial condition.
