About The Position
The Home Health and Hospice Aide is a member of the home care team who works under the supervision of a registered nurse or therapist to provide personal care services that meet patients’ needs. The aide observes patients, reports observations, and documents observations and care performed. Assignments are made to promote quality, continuity, and safety of patient care, and the aide may be assigned only after successfully completing a competency evaluation program. The aide is also part of the interdisciplinary team. Responsibilities
- Perform personal care services as necessary to meet patients’ needs.
- Observe patients and report observations.
- Document observations and care performed.
- Work as part of the interdisciplinary home care team under the supervision of a registered nurse or therapist.
- Complete a competency evaluation program before being assigned to patient care.
