About The Position
The Home Health and Hospice Aide is a member of the home care team and works under the supervision of a registered nurse or therapist. The aide provides personal care services as needed to meet patients’ needs, observes patients, reports observations, and documents observations and care provided. Assignments are made to promote quality, continuity, and safety of patient care, and occur only after successful completion of a competency evaluation program. The aide is also part of the interdisciplinary team.
