Corewell Health Hospice specializes in providing end-of-life care and symptom management to individuals with a life expectancy of six months or less. They serve individuals in their homes, nursing facilities, and hospitals. In collaboration with the entire interdisciplinary team, the CNA will assess, plan, implement, coordinate, monitor, and evaluate the strategies and services required to meet a patient/family's end-of-life care needs. An outcome-based plan of care is created and continually adjusted through utilization of the nursing process, relationship-centered care principles, and a holistic philosophy, assuring the changing needs of the patient/family are met while facilitating a symptom-free, peaceful, sacred death.
