About The Position

The Care Team Hospice is seeking a CNA to provide personal care and comfort-focused support to hospice patients in their homes and in facility settings, working alongside our interdisciplinary team to ensure dignity, compassion, and quality of life throughout the hospice journey. Requirements

• Active CNA certification
• Active driver’s license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
• Current CPR certification as required for the role

Nice To Haves

• Hospice aide experience preferred
• Experience using HCHB (Homecare Homebase) preferred

Responsibilities

• Provide personal care, hygiene, and comfort measures
• Observe and report changes in patient condition
• Support patients and families with compassion and respect
• Document care provided accurately and timely in the EMR HCHB (Homecare Homebase)
