About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Per diem CNA Hospice Aide provides compassionate hands-on care to hospice patients and supports their families during end-of-life care. The role assists patients with daily living activities while promoting comfort, dignity, and emotional well-being. Working with the hospice care team, the aide provides respectful, individualized care in accordance with each patient’s care plan.

Requirements

• Current state certification or registration as a nurse assistant (CNA), in accordance with state requirements.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and liability insurance.

Nice To Haves

• CNA experience within the last two years.

Responsibilities

• Provide personal care services to patients.
• Apply safety principles and proper body mechanics when assisting with ambulation, transfers, range of motion, positioning, and household chores.
• Plan and prepare nutritious meals and assist with feeding, as assigned.
• Perform homemaking and other environmental services, as assigned.
• Observe, report, and document changes in a patient’s status; report to the primary nurse if unable to perform a task.
• Follow specific written personal-care instructions prepared by a registered nurse in accordance with the plan of care; receive the instructions before delivering services.
• Assist patients with prescribed exercises as instructed by a nurse or therapist.
• Perform and record simple urine tests for sugar, acetone, and albumin.
• Perform other duties as assigned.
