About The Position

Your work family will include doctors, nurses, social workers, CNAs, chaplains, and volunteers. As a Hospice Aide/Certified Nursing Assistant (CNA), you are a crucial part of the Interdisciplinary Team, serving as the heart, eyes, and key communicator of patient needs. Your role involves following a personalized Care Plan for each assigned patient and collaborating closely with the RN Case Manager to ensure patient care meets and exceeds expectations. You will interact effectively with all team members, patients, and their loved ones, upholding the highest standards of care and professional clinical competency.

Requirements

Current CNA License in the State of Georgia
Current CPR & First Aid Certification
1-2 years hospice experience (other candidates may be considered with experience in assisted living, home health, long term care)
Utmost compassion & integrity
Driver’s license, insurance and vehicle
Excellent Time Management Skills

Responsibilities

Follow a carefully planned out, personalized Care Plan for each patient.
Report and work closely with the RN Case Manager to ensure the care of the patient is completed and expectations are exceeded.
Effectively interact with all members of the Interdisciplinary Team as well as with your patients and their loved ones.
Maintain the highest standards of care and professional nursing clinical competency.
