About The Position

The CN LPN provides direct nursing care to residents at the Center at Centennial in Colorado Springs, Colorado. The role includes medication administration, wound care, vital sign monitoring, assessing and reporting changes in residents’ conditions, contributing to care plans, and documenting care in electronic health records. The LPN collaborates with physicians, nursing staff, and other departments to support comprehensive resident care and participates in facility surveys and quality improvement initiatives. The position works in a fast-paced environment with frequent interruptions and may involve exposure to infectious waste, diseases, and hazardous chemicals; PPE is provided. The role may require working beyond normal hours, including weekends and holidays.

Requirements

Hold a current, unencumbered LPN license.
Ability to make independent decisions and exercise sound judgment.
Strong communication and interpersonal skills, including the ability to interact tactfully with residents, families, and staff.
Knowledge of nursing and medical practices, procedures, and long-term care regulations.
Ability to work effectively in a team environment and supervise other personnel as needed.
Patience, compassion, and a positive attitude.
Ability to lift, push, pull, and transport residents with assistance as needed.

Responsibilities

Provide direct nursing care to residents, including medication administration, wound care, and vital sign monitoring.
Observe and assess residents, and report changes in their condition to physicians and the nursing team.
Assist in developing and modifying resident care plans.
Document resident care accurately and thoroughly in electronic health records.
Collaborate with physicians, nursing staff, and other departments to ensure comprehensive resident care.
Participate in facility surveys and quality improvement initiatives.
Maintain a safe and therapeutic environment for residents.
