About The Position

Reporting to the program manager, the CMHC, Integrated Seniors will be responsible for supporting adults over the age of 55 experiencing complex physical, mental health, and medical concerns, who may also be without appropriate housing. Their main goal is to provide education and support to clients to address issues of isolation, safety, crisis prevention, and overall wellness.

Requirements

  • Bachelor’s Degree or Diploma in social work, nursing, gerontology; or a diploma in practical nursing or social services
  • 2+ years’ experience of case management experience or equivalent
  • Working knowledge of the provision of health care in a variety of settings is essential
  • Knowledge of community resources is required
  • A clear understanding of the characteristics and problems of seniors with mental illness, and be knowledgeable about the various service models used to serve this populations is essential
  • Knowledge of various computer programs i.e. Word, Excel, MS Teams, Outlook and the ability to learn new software is required
  • Knowledge of MS Office Suite (Word, Teams, Excel, Power Point and Outlook)
  • Excellent assessment, risk management, and consultation skills
  • A valid Ontario Class “G” Driver’s license and a vehicle with adequate insurance coverage is required
  • Valid certification in CPI, CPR, First Aid, and Assist is mandatory

Responsibilities

  • Interview clients and their families in the appropriate care setting
  • Obtain and document assessment information utilizing approved procedures, forms, and consents
  • Identify high risk factors, barriers to goals, and strengths that will assist the client in achieving goals
  • As appropriate, develop an accurate, written, care plan which identifies a variety of methods to meet the client needs
  • Identify community resources and incorporate them into the plan
  • Complete assessments to guide and support service, such as, OCAN, InterRAI Cha, falls screener, risk assessment
  • Develop a comprehensive case management plan that addresses all pertinent areas of the assessment
  • Support with housing stability
  • Provide clients, caregivers, and health service providers with appropriate supplementary information as needed
  • Act as a client advocate with care givers, families or agencies providing care
  • Attending meetings as scheduled
  • Follow organizational Infection Prevention and Control policies and procedures, and safe use of Personal Protective Equipment at all times
  • Work from an equity, diversity, and inclusionary lens with clients and colleagues
  • Understand and be able to integrate a philosophy of client-centred care into daily responsibilities
  • Demonstrates a strong commitment to quality care and service delivery based on SHIP’s Mission and Values
  • Ensure appropriate records are kept and make sure that statistical information is maintained as per agency standards
  • Adhere to agency Health & Safety Program and the Client Safety Framework in the promotion of client safety
  • Perform additional duties as required/delegated by Management
  • You may be required to administer naloxone in response to suspected opioid overdoses, and must complete appropriate training to recognize overdose symptoms, safely administer naloxone via nasal spray or injection, and follow emergency protocols including contacting emergency services. You will receive training within the first eight weeks of employment.
