This is a full-time, non-exempt position within the Human Services & Health Department. The CLTS Case Manager works to achieve involvement and participation of the client, client's family, and client's natural support system in addressing and resolving problems. The role involves developing individual service plans, protecting children, maintaining family units, and preventing out-of-home placement whenever possible. The Case Manager assesses and intervenes in crisis situations, refers to community resources, and coordinates access to services. They also teach clients about community resources, advocate for services, and maintain case records. The position requires developing and maintaining professional knowledge and skills, consulting with supervisors and interdisciplinary teams, and potentially engaging in public education. Other duties as assigned.
