About The Position

Health Solutions is seeking a Clinician to join its Youth & Family Services team in Pueblo, Colorado. The role is part of a multidisciplinary team promoting healthy youth and families through outpatient and home-based services. The Clinician provides case management and outpatient therapy for individuals and families, supporting clients, their families, and the community. The position is supervised by the Director and Supervisor and involves collaboration with community providers and health care professionals. Some evening and on-call hours are required. Travel is up to 25%.

Requirements

Graduate degree in social work, psychology, or a closely related field.
Demonstrated computer proficiency and experience with electronic health records.
Colorado driver’s license and access to transportation during work hours.
Must already be authorized to work in the United States; sponsorship is not available.

Nice To Haves

Colorado professional license (such as LPC, LCSW, or LMFT), or license eligibility.
At least one year of mental health care experience treating children or adolescents.
Previous experience working in a home-based program.
Spanish language skills.

Responsibilities

Provide intake and assessment, including professional interpretation and clinical analysis, to develop treatment plans.
Provide case management and collaborative, integrative services with community resources.
Assess and develop treatment plans for clients seeking residential treatment.
Monitor client placement, support discharge planning, and identify step-down services for youth and their families.
Maintain accurate, complete, and up-to-date records in compliance with Center standards.
Maintain competency and proficiency with the agency’s Electronic Health Record.
Collaborate with community providers and health care professionals.
Perform other job-related duties as assigned by a Program Director or Supervisor.
