About The Position
The Clinician Case Manager is responsible for direct services with children, adolescents and their families including but not limited to evaluation/assessment; home & community based services; outreach; case and care management; individual and family therapies; crisis intervention; record keeping; collaboration with other professionals and team members and related community services (including consultation and/or community and/or school placements). He/she may be assigned to carry a pager and responds to all calls for assistance either by telephone or in person (as clinically indicated).
Requirements
- Master’s Degree in marriage and family therapy, social work, or related human services field with at least a one-year clinical internship working with children and/or adolescents.
- A Connecticut Associates license is minimum requirement; (LPC-A, MFT-A, LMSW).
- Valid driver’s license, reliable transportation and valid auto insurance required.
- CPR and first-aid certification.
Nice To Haves
- A Connecticut LMFT, LPC, LCSW or Doctorate Psychologist preferred.
- Two years post graduate clinical experience working with children, adolescents, families or providing crisis intervention is preferred.
- Experience in group and family therapies is also preferred.
- Bi-lingual (Spanish-English) may be required.
Responsibilities
- Provide comprehensive behavioral health services to children, adolescents, and families, including assessments, treatment planning, individual, group, and family therapy, crisis intervention, case management, and community-based outreach services.
- Deliver services in a variety of settings such as homes, schools, hospitals, shelters, and community locations.
- Implement treatment plans and coordinate supportive services.
- Maintain ongoing contact and follow-up with clients and families.
- Ensure timely and confidential documentation.
- Participate in after-hours emergency coverage as assigned.
- Maintain productivity standards.
- Collaborate with multidisciplinary teams and community partners.
- Participate in trainings, supervision, staff meetings, and committees.
- Assist with program development and implementation efforts.
- Supervise assigned staff or interns as applicable.
- Perform additional duties as assigned by leadership.
