About The Position

Greystone Foster Care, in partnership with Universal Health Services, Inc., is seeking a dedicated and compassionate Clinical Therapist to join their growing team. This hybrid role involves providing in-home therapy services to youth in foster or birth homes, developing treatment plans, conducting assessments, and coordinating care. The position requires flexibility for evenings and weekends, as well as participation in an on-call rotation. The ideal candidate will reside in the Southern/Middle TN area (Davidson, Rutherford, Wilson, Bedford, Coffee, and Cannon counties).

Requirements

  • Master's degree in Social Work or related field.
  • Three to five years' related experience and/or training, or equivalent combination of education and experience.
  • Maintain procedures to build effective relationships with families and agencies.
  • Ability to think and act quickly and calmly in an emergency and make independent decisions.
  • Excellent leadership, communication, and organizational skills.
  • Valid state Driver's License.
  • Automobile insurance with verification of medical liability.
  • Successful background check, fingerprinting and employment verification.

Responsibilities

  • Provide in home therapy services for level 3 youth in the foster or birth home at least once per week.
  • Within 30 days of placement, develop Master Treatment Plans upon admission and updates in conjunction with the treatment team.
  • Meet/Greet new admits within 24 hours of admission.
  • Conduct Psychosocial assessments with youth upon 72 hours of admission.
  • Coordinate activities to ensure that treatment plan goals and objectives are met.
  • Maintain accurate, complete, timely, and appropriate clinical records and other required information.
  • Complete discharge planning to include recommendations, approval from guardians, and at times, arranging discharge plans.
  • On-Call Rotation when needed by the facility to meet the requirements of all governing agencies.
  • Collaborate with other staff in coordinating therapeutic services.
  • Performs other duties as assigned/required by this position.
