About The Position

This position provides direct service to patients in a Behavioral Health Program area. The role involves completing psychosocial assessments and treatment plans in cooperation with the patient and a multidisciplinary team. A comprehensive assessment of the clinical needs of each patient is conducted prior to therapy. The therapist works collaboratively with the patient to develop a treatment plan addressing their identified goals and delivers behavioral health care in accordance with the patient's evaluation, treatment plan, and within the scope of their license. This includes working directly in a treatment relationship with patients, individually or in groups, in consultation with a Psychiatrist and/or NP. The role also involves providing crisis assessment and intervention, consulting and coordinating patient care with physicians, NPs, therapists, and community agencies, and providing appropriate discharge planning and referrals to community resources. Participation in clinical collaboration and/or psychiatry supervision is required to discuss cases and determine appropriate treatment courses. The therapist is responsible for maintaining pertinent patient data and documentation in the medical record according to state, TJC, and Mercyhealth system guidelines. Additionally, the role requires attending required meetings or staffing as indicated by the Program Manager, participating in performance improvement activities, and performing other duties as assigned. Compliance with all Mercy Health System, State, and JCAHO policies and regulations is mandatory.

Requirements

Master's degree in counseling, Social Work, Psychology or other related field
Must possess all required certification and licensure per the state in which they will be practicing.
Wisconsin: LCSW, LPC, or LMFT
Illinois: LCSW, LCPC, LMFT

Nice To Haves

TACT Level 2
CPR/BLS preferred
Dual Licensure Wisconsin: Wisconsin Substance Abuse Counselor--SAC or Clinical Substance Abuse Counselor--CSAC
Illinois: CADC or CRADC

Responsibilities

Completes psychosocial assessments and treatment plans in cooperation with patient and multidisciplinary team.

Consults and coordinates care of patient with physician and or NP, therapists, and community agencies.
Provides appropriate discharge planning.
Refers patient or patient's family to other community resources.
Participates in clinical collaboration and or psychiatry supervision to discuss cases and determine appropriate course of treatment.
