Manages, supervises, coordinates, evaluates, and develops client care teams to ensure quality care delivery within the scope of services of the Agency and all applicable policies, standards, laws, and state Nurse Practice Act. Requirements

  • Graduate of an accredited school of professional nursing. BSN preferred.
  • Current license as a Registered Nurse in the state(s) of practice with at least two (2) years of homecare experience and one (1) year of supervisory experience.
  • Excellent verbal and written communication skills and strong interpersonal skills.
  • Must be a licensed driver with an automobile insured in accordance with Agency and state requirements.
  • Current CPR certification.

  • BSN preferred.

Responsibilities

  • Conduct in-home client visits such as start-of-care assessments, supervisory visits, and reassessment visits.
  • Be available during business hours and on-call as needed to support quality care delivery.
  • Receive referrals, assess service needs, and assign appropriate clinicians to cases.
  • Meet regularly with clinical staff to provide guidance, set goals, and evaluate progress.
  • Attend clinical meetings to facilitate care coordination and participate in on-call rotation.
  • Support staff, clients, and families by providing direction on healthcare resources and educating them on clinical policies.
  • Oversee accuracy and timeliness of clinical documentation and perform quarterly chart reviews.
  • Assist with staff hiring, orientation, training, and conducting annual performance evaluations.
