About The Position

James River Home Health and Hospice has partnered with Hueman to find a Clinical Supervisor for their Hospice Care Service team located in Richmond, VA. The Clinical Supervisor is responsible for coordinating and managing patient care services to ensure quality outcomes. This role supports the Director of Clinical Services in maintaining excellence in patient care in alignment with agency policies, regulatory requirements, and organizational objectives. The Clinical Supervisor plays a key role in planning, implementing, and evaluating hospice services while ensuring compliance and staff development. Requirements

  • Graduate of an accredited School of Nursing.
  • Current Registered Nurse (RN) license in the state of Virginia.
  • Minimum of 2 years of nursing experience in home hospice or skilled home care (hospice experience preferred).
  • Knowledge of healthcare regulations, ACHC standards, and business management practices.
  • Strong verbal and written communication skills.
  • Ability to demonstrate leadership, professionalism, and a positive attitude.
  • Capable of building rapport and interacting effectively with patients, families, staff, and the community.

Responsibilities

  • Coordinate and oversee all direct and indirect patient care services provided by clinical staff.
  • Ensure the delivery of high-quality care and adherence to clinical standards.
  • Review initial intake screenings to assess patient needs and determine appropriateness for home care services.
  • Conduct patient visits in the home setting as needed.
  • Provide supervision, mentorship, and guidance to clinical personnel.
  • Evaluate staff performance during probationary periods and annually.
  • Deliver coaching, counseling, and training to enhance professional development and service delivery.
  • Assist with onboarding and orientation of new hires.
  • Facilitate ongoing education and in-service training for clinical staff.
  • Perform regular chart audits to ensure documentation accuracy and compliance.
  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
  • Ensure compliance with Medicare, State regulations, and ACHC standards.
  • Support the Director of Clinical Services with regulatory and operational responsibilities.
  • Maintain adequate levels of medical supplies and ensure proper inventory management.
  • Assist with administrative and clinical operations as delegated.
  • Promote positive relationships with patients, families, staff, and community partners.
  • Maintain current clinical knowledge and management skills through continuing education, workshops, conferences, and professional engagement.
  • Demonstrate a commitment to excellence, continuous improvement, and leadership development.
