Requirements

Diploma, Associae or Baccalaureate Degree in nursing
1+ year home health experience
Must have a current, unrestricted RN license in the state you are working, may be required to obtain additional state licensure.
Demonstrates strong leadership, effective communication and interpersonal skills.
Ability to travel 50% travel required (mainly to branches in your region)
Proficiency with OASIS

Nice To Haves

Previous nurse preceptor or educator experience is preferred
Proficiency with HomeCare HomeBase & PointCare is preferred.

Responsibilities

Act as a liaison and maintain communication with new hire and current clinicians, branch leadership, and preceptors to provide practical clinical experience and guidance to field clinicians, including standardized orientation of new clinicians.
Ensure execution of the new hire field clinician orientation experience via: Preceptor Guide Competency Checklist State specific requirements as needed
Demonstrate processes for SOC, ROC, Recert and routine visits to clinicians during orientation process
Work with preceptors and/or clinicians to review application of clinical documentation that meets regulatory and compliance standards and guidelines
Provide education to ensure effective collaboration with healthcare providers in accordance with physician’s orders and plan of care.
Continually work to create a culture of learning and support for preceptors and new clinicians as they are gaining confidence and competence in their respective role.
Meets with Preceptors and/or Orientees on regular basis to assess progress during Orientation.
Communicates with CTMs and/or BDs as needed to discuss progress of Orientees.
Collaborate with preceptors and local leadership team to determine & participate in the delivery of targeted education based on needs and process improvements to align with Elara goals and vision
Facilitate annual skills lab & competency assessments as needed
Other duties as assigned to support the changing needs of the business.
